  Employment Income T-slips: T4, T4A or other employment T-slips
Employment Insurance T4E slip
Pension Income T4A-slips: T4A (Pension), T4AP (Canada Pension), and T4AOAS (Old Age Security)
Investment Income T-slips: T4RSP (retirement fund) T4RIF (income fund), T3, or T5
Social Assistance T5007 slip. Note: if married or common-law, must be claimed by the higher income spouse.
Workers Compensation T5007 slip or T4A indicating Workers Compensation or Wage Loss Replacement.
Universal Child Care Benefit (UCCB) RC62. Note: if married or common-law, must be claimed by the lower income spouse.
Child Support (received) Provide the amount if payment is required by a court order dated before May 1 1997.
Spousal Support (received) Provide the amount if payment is required by a written agreement.
Spouse’s Income Money Mart must prepare both yours and your spouse’s return at the same time unless:
• Your Spouse has no income or doesn’t live in Canada
• You provide a T1 Summary of your Spouse’s return
• You and your spouse have no dependants and both earned over $15,000 in the tax year
  Home Renovation Tax Credit All qualifying receipts. If you are making a claim for labour costs a GST number for the contractor is required.
Union or Professional Dues Official tax receipts required if not on T4.
Public Transit Passes Monthly or weekly passes.
Medical Expenses Payment receipts or insurance statement.
RRSP Contributions Official tax receipts required if not on T4.
Child Care Expenses Receipts of payment .
Donations Official tax receipts required.
Tuition and Textbooks T2202A from school .
Child Support (paid) Receipts of payment and a copy of Child Custody Agreement .
Spousal Support (paid) Receipts of payment and copy of Spousal Support Agreement.
Rent (MB and ON only) Statement of Rent Paid from Money Mart signed by your landlord, or Receipts for each month .
